Regarding his middle name. There is an Ohio birth record court "Journal Entry", dated 1942, giving his birth date, plus his middle name as "Ellisworth". His middle name on his daughter's (Mary) birth is given as "Ellsworth". Given there are known factual errors in the journal entry (misspelled his mother's middle name), it is difficult to know the true spelling.
