There are three separate records under "United States, National Homes for Disabled Volunteer Soldiers, 1866-1938". Apparently one starts where the other leaves off. All three are pictured, most recent first.

All three have this information:
Enlisted: Corinth, Mississippi; Date of enlistment: 20 March 1863; Date of discharge: 02 July 1863; Cause of discharge: Disability; Rank: Pvt; Company D, 1 Alabama Cavalry; Born: Alabama; Occupation: Physician; Marital status: Widower; Residence since discharge: Oklahoma City, Oklahoma; Pension Certificate: 1069057.

All three records mention daughter, Ida L Davis. The record from Sawtelle, California mentions a second daughter, Mrs Nora B Landers and the record from Leavenworth, Kansas mentions a brother, G M Masterson. The Leavenworth, Kansas record also has a notation in pencil that John's effects were given to widow, Ann Masterson.
