To correct a couple of errors in other published accounts, information from Mr. Williams' passport application has been used in this memorial. First, his birth place: Mr. Williams recorded his birthplace as the State of Maine. (He was raised in Taunton, Bristol County, Massachusetts, which has been erroneously reported as his birthplace.) Second, the spelling of his middle name: He signed his middle name "Maxey".

His first burial place was Laurel Hill Cemetery.
