He was born July 15, 1947, in Victoria to Fred Slavik of Port Lavaca and the late Alma Elma Lesikar Slavik. He was a member of Northside Baptist Church.
Survivors: father; sister, Evelyn Hoffman of Ganado; and brothers, Julius Slavik of Corpus Christi and Bobby Ray Slavik and George Anthony Slavik, both of Port Lavaca.
Preceded in death by: brother, James Fred Slavik.
Graveside services: 1 p.m. today, Evergreen Cemetery, the Rev. Mickey Ewing officiating.
